IMPORTANT INFORMATION for PSEMAS Members

As from 18 July 2005, all members of PSEMAS and their dependants receiving medical services, as well as the providers of Medical services, are required to fill in a standard form of declaration of any medical services attained. This form should accompany any claim submitted for payment.
